What is Pain Management?

Pain Management is the act, manner or practice of managing pain, handling or control of an unpleasant sensation occurring in varying degrees of severity as a consequence of injury, disease, or emotional disorder. Treatment for chronic pain is best understood and accomplished by a physician specializing in pain management.

Do I need to obtain a referral/prior authorization from my MD?

No, obtaining a referral from your MD is not required, unless specified by your insurance company. However, referrals are highly recommended and graciously welcomed.

What do I need to do to become a new patient?

  1. FSC requires you to provide us with the last 2 months of medical records associated with your pain problems. If none are available just kindly indicate that when you call to make your initial consultation.
  2. If you have previously seen a pain management physician prior to your initial consultation appointment, we require written notification from that particular physician that he or she will no longer be prescribing you pain medication and is turning over all treatment, in relation to pain management, to FSC.
  3. Fill out the form(s), that correspond to your specific needs. If you are unable to complete/print this form and bring to your initial consultation, then we request that you arrive approximately 30 minutes prior to your appointment to allow adequate time for completion.

What is the best way to transfer my medical records to/from FSC?

Fill out the Medical Records Authorization Form. Complete the form according to your needs and deliver to the proper facility. Please allow 7-14 business days from your request to be processed.
